This project was developed to fill knowledge gaps about individuals living with multiple chronic conditions— including more detailed characterization of the population and the impact of this disease on the individual, the population as a whole and the health care system. [...] Using health administrative data for multiple chronic disease surveillance Accurate and timely measurement of the burden of chronic diseases is the cornerstone of sound evidence-based health care planning. [...] Thus, the accuracy of health administrative data depends not only on the quality of the data but also the specific condition being identified and the validity of the codes in the patient group (7). [...] In response to the identified need for more rigorous assessments of the quality of administrative data, the Institute for Clinical Evaluative Sciences developed the Multiple Chronic Disease in Canada. [...] The first, or Disease Table, is used to ascertain disease status and has three data elements: the unique encoded personal identifier, the disease status, and the date of diagnosis for the disease.
