Document t [...] Measures for Objective 3 In the analysis of one-year outcomes of persons discharged home the follow- ing six categories of outcomes were reported: death, admission to PCH, admission to a long-term care hospital (Deerlodge, Riverview), receipt of home care, rehospitalization and none of the above.
